Responsibilities:
We are seeking a Care Transition Specialist to support our Outcomes Department by completing post-discharge phone surveys within 24–72 hours of a patient’s discharge. In this role, you will interview patients, document their status in our database, and identify potential risk factors for readmission. This position does not provide medical advice; instead, you will use strong communication skills to assess patients' health status and well-being post-discharge.

- Conduct post-discharge phone surveys within 24–72 hours of patient discharge and complete assessments in the JLM database.
- Verify patient demographics and discharge information to ensure accurate documentation.
- Document patient-reported concerns, symptoms, barriers, and follow-up needs clearly and thoroughly.
- Identify and escalate unresolved issues related to health or care coordination according to established protocols (without providing medical advice).
- Communicate trends, training needs, and process improvement ideas to management and contribute to preventative care initiatives.
- Participate in defining and implementing new processes, standards, and technology that improve patient experience and outcomes.
- Maintain compliance with all applicable regulatory requirements, standards, and internal procedures.
- Perform other duties as assigned.
Qualifications:
- Active, unrestricted RN or LPN license, or previous medical/clinical training.
- Associate degree (or higher) in Nursing or a related clinical field.
- Strong listening, verbal communication, and customer-service skills with a patient-centered approach.
- Ability to rely on experience and judgment to plan and accomplish goals while handling multiple tasks concurrently.
- Proficiency in medical terminology with the ability to produce accurate clinical documentation and maintain strong spelling and grammar.
Preferred Qualifications:
- Familiarity with psychiatric/behavioral health concepts, practices, and procedures.
- Experience conducting patient follow-up calls, care coordination, discharge planning, or patient satisfaction/experience work.
Schedule & Work Arrangement:
This role offers a flexible schedule with adjustable work times and workdays based on department needs and individual availability.
